A Resolving Chaos-led project has made it to the last round of the Local Government Chronicle (LGC) Awards.
Building on our work in Lambeth, Southwark and Lewisham we teamed up with South London and the Maudsley NHS Trust to create the You Can team supporting people with multiple needs in the district council areas of Three Rivers and Hertsmere.
Supported by the county and district councils, local police, NHS and charity partners the programme has improved many people's lives and saved the taxpayer money by taking the strain off emergency services. Such is the success that the Local Government Chronicle has shortlisted the programme for a prestigious Health and Social Care Award.
You Can project manager Bianca Hegde said: “it’s a real honour to be shortlisted for the LGC award. The project is innovative on many levels, from the way it has been funded and commissioned, to the project governance and partnership to the way we are working with participants on the ground. It’s great to be part of a partnership that is forward thinking and trying to do things differently for the most complex vulnerable in the community.”
